Summary

Incisional negative pressure therapy (INPT) significantly reduced donor-site complications after anterolateral thigh (ALT) flap surgery. This approach is particularly beneficial for larger ALT flaps, decreasing wound healing issues.

Background:

  • Primary closure of anterolateral thigh (ALT) flap donor sites can lead to significant morbidity.
  • Incisional negative pressure therapy (INPT) is a potential method to reduce complications in high-risk surgical incisions.

Purpose of the Study:

  • To evaluate the efficacy of INPT in reducing donor-site complications following primary closure of ALT flaps.
  • To compare complication rates between ALT donor sites treated with INPT versus traditional dressings.

Main Methods:

  • A retrospective cohort study analyzed patients undergoing ALT free flap reconstruction with primary donor-site closure.
  • Patients were divided into two groups: those treated with INPT (study group) and those with traditional dressings (control group).
  • Nonparametric statistics were used to identify prognostic factors, with a significance level of p < 0.05.

Main Results:

  • The INPT group (n=28) had a global complication rate of 7.14%, compared to 37% in the control group (n=30) (p=0.007).
  • INPT was associated with significantly lower rates of wound dehiscence and skin necrosis (p < 0.05).
  • Multivariate analysis confirmed INPT significantly reduced donor-site complications (p=0.006), especially for defects larger than 8 cm (p=0.003).

Conclusions:

  • Incisional negative pressure therapy significantly decreases donor-site morbidity after ALT flap harvest.
  • INPT is an effective adjunctive therapy for primary closure of ALT flap donor sites, particularly in cases with larger defects.
