A pulse is a short burst of radio waves distributed over a range of frequencies that simultaneously excites all the nuclei in the sample. Upon passing a radio frequency pulse along the x-axis, the nuclei absorb energy corresponding to their Larmor frequencies and achieve resonance. This shifts the net magnetization vector from the z-axis toward the transverse plane. This angle of rotation of the magnetization vector, or the flip angle, is proportional to the duration and intensity of the pulse.

    Background:

    • Radio-frequency ablation (RFA) is an emerging technique for treating thoracic conditions.
    • Videothoracoscopic operations (VTHO) offer a minimally invasive approach to thoracic surgery.
    • Lung cancer and malignant pleural effusion present significant clinical challenges.

    Purpose of the Study:

    • To evaluate the efficacy and safety of RFA in VTHO for patients with primary or metastatic lung cancer and malignant pleural effusion.
    • To assess the outcomes in patients unsuitable for radical surgical intervention.

    Main Methods:

    • RFA was performed using VTHO in 56 patients.
    • Patient cohort included primary lung cancer (n=7), metastatic lung cancer (n=12), and malignant pleural effusion (n=37).
    • Histological subtypes included small-cell, squamous-cell, and glandular lung cancer.

    Main Results:

    • Positive treatment results were achieved in 54 out of 56 patients.
    • Average duration of pleural drainage was 3 days, with an average hospital stay of 4 days.
    • Survival data showed promising results, with 25 patients with metastatic pleurisy surviving over 9 months.

    Conclusions:

    • VTHO with RFA is a potentially effective and minimally invasive treatment for selected patients with lung cancer and malignant pleural effusion.
    • The procedure demonstrates favorable short-term outcomes regarding recovery time.
    • Further long-term follow-up is necessary to fully ascertain the late outcomes of this therapeutic approach.
