This study developed a collaborative multidisciplinary team care (MTC) service model to prevent drug-related harm. The DrugTEAM service, designed using a service design framework, addresses stakeholder needs for better patient outcomes.
Area of Science:
- Healthcare Management
- Patient Safety
- Service Design
Background:
- Drug-related morbidity and mortality remain significant public health concerns.
- Existing patient care services often have unmet needs and stakeholder perspectives are not fully addressed.
- A collaborative approach is essential for effective medication management.
Purpose of the Study:
- To develop a collaborative multidisciplinary team care (MTC) service model to prevent drug-related morbidity and mortality.
- To address unmet needs and perspectives of diverse stakeholders using a service design framework.
- To create a patient-centered service for improved drug therapy evaluation and management.
Main Methods:
- Utilized a 4D service design framework (Discover, Define, Design, Develop).
- Conducted stakeholder research (desk, field, focus groups, interviews) to identify needs.
- Developed and validated four service prototypes: medication reconciliation, evaluation/management, drug information, and care transitions.
Main Results:
- A novel service concept, Patient-oriented, Collaborative, Advanced, Renovated, and Excellent (P-CARE), was defined.
- The "DrugTEAM" (Drug Therapy Evaluation And Management) service model was developed.
- Four key service components were designed and implemented within a chronic care model.
Conclusions:
- The developed collaborative MTC service model effectively addresses unmet needs in drug therapy management.
- Implementation of the "DrugTEAM" service is expected to enhance pharmacist-stakeholder relationships.
- The model aims to ultimately improve patient outcomes and reduce drug-related harm.
