Background:

  • Iridocorneal syndromes are a heterogeneous group of diseases characterized by endothelial cell proliferation over the trabecular meshwork.
  • This proliferation obstructs aqueous outflow, leading to elevated intraocular pressure (IOP).

Observation:

  • A case of suspected ciliary body melanoma presented with a pigmented iris root lesion and severe ocular hypertension unresponsive to medical therapy.
  • Diagnostic workup revealed unilateral corneal endothelial changes consistent with an iridocorneal syndrome.

Findings:

  • Surgical intervention with an Ahmed valve implant successfully achieved sustained IOP control.
  • This management also led to the resolution of visual field defects.

Implications:

  • Tube shunt surgery, such as Ahmed valve implantation, may represent a primary treatment modality for refractory iridocorneal syndromes.
  • Comprehensive diagnostic evaluation is essential to differentiate iridocorneal syndromes from other potentially sight-threatening and systemic conditions.
