    Summary

    Elevated cyclic guanosine monophosphate (cGMP) levels are linked to atrial fibrillation (AF) development and persistence. This study found increased c-GMP during induced AF in dogs and patients, suggesting its crucial role.

    Background:

    • Atrial fibrillation (AF) is a common arrhythmia.
    • The role of cyclic nucleotides, like cyclic GMP (cGMP) and cyclic AMP (cAMP), in AF pathogenesis is not fully understood.

    Purpose of the Study:

    • To investigate the involvement of cyclic nucleotides in the initiation and maintenance of atrial fibrillation.
    • To determine the effects of cGMP on the atrial fibrillation threshold (AFT).

    Main Methods:

    • Measured plasma cGMP and cAMP levels in dogs with electrically induced AF and during high-frequency atrial pacing.
    • Determined AFT after administering a cGMP derivative (Dbc-GMP) and assessed the effect of atropine.
    • Measured plasma cGMP and cAMP levels in human patients with paroxysmal AF and sinus rhythm.

    Main Results:

    • Plasma cGMP levels significantly increased during induced AF in dogs and during high-rate pacing.
    • Administration of Dbc-GMP dose-dependently decreased AFT in dogs.
    • Patients with AF exhibited significantly higher plasma cGMP levels compared to those in sinus rhythm.
    • cAMP levels remained largely unaffected in all experimental conditions.

    Conclusions:

    • Increased cGMP levels play a significant role in the genesis and/or persistence of atrial fibrillation.
    • cGMP, but not cAMP, is implicated in the pathophysiology of AF.
