    Background:

    • Measuring local IgG response in ocular Herpes simplex virus (HSV) infection is challenging due to limited tear samples and potential serum IgG transudation.
    • Herpes simplex keratitis involves inflammation of the cornea caused by HSV.

    Purpose of the Study:

    • To analyze the local IgG antibody response in Herpes simplex keratitis.
    • To investigate the potential role of specific human IgG subclasses in ocular mucosal immunity.

    Main Methods:

    • Enzyme-linked immunosorbent assay (ELISA) using specific monoclonal antibodies to Human IgG subclasses.
    • Radioimmunoprecipitation assay (RIPA) of HSV antigens.

    Main Results:

    • All serum samples (patients and controls) had quantifiable HSV-specific IgG1 and IgG4.
    • HSV-specific IgG1 serum concentrations were significantly higher (≥16.1-fold) than in tears, while IgG4 was only 6.5-fold higher.
    • Minor differences were observed in protein profiles between serum and tear antibody immunoprecipitation.

    Conclusions:

    • The differential concentrations of IgG1 and IgG4 in serum versus tears suggest a specific role for IgG4 in ocular mucosal immunity.
    • These findings support the hypothesis that IgG4 antibodies contribute to mucosal defense in the eye, similar to their proposed role in the lung.
