    Summary

    Fasting reduces serum angiotensin-converting enzyme (ACE) activity, independent of thyroid hormone (T3) levels. Blood pressure drops during fasting may be linked to low T3, not reduced ACE activity.

    Background:

    • Serum angiotensin-converting enzyme (ACE) activity correlates with thyroid hormone levels.
    • Fasting leads to decreased triiodothyronine (T3) levels, prompting an investigation into its effect on ACE activity.

    Purpose of the Study:

    • To determine if serum ACE activity decreases during fasting due to reduced T3 levels.
    • To investigate the relationship between fasting, T3 levels, ACE activity, and blood pressure.

    Main Methods:

    • Eighteen obese euthyroid subjects were studied.
    • Group 1: 8-day fast (50 kcal/day).
    • Group 2: 8-day fast with T3 supplementation (5 micrograms q 3 h).
    • Measurements included serum T3, blood pressure, ACE activity, glucose, and insulin.

    Main Results:

    • Weight loss was similar in both groups (-4.4 kg).
    • Serum T3 decreased significantly in Group 1 but remained unchanged in Group 2.
    • Blood pressure decreased significantly in Group 1 but not in Group 2.

    • ACE activity reduction in Group 2 occurred later in the fast, irrespective of T3 supplementation.

    Conclusions:

    • Fasting reduces serum ACE activity, an effect not mediated by T3.
    • Fasting-induced blood pressure reduction may be related to low T3 levels, not decreased ACE activity.
    • Serum ACE activity interpretation requires consideration of dietary context.
