Postconditioning with lactate-enriched blood (PCLeB) is a novel cardioprotection strategy. Muscle squeezing in the left anterior descending artery after reperfusion is not rare in anterior ST-segment elevation myocardial infarction patients treated with PCLeB.

Purpose of the Study:
- To determine the prevalence of coronary artery muscle squeezing after reperfusion in anterior STEMI patients treated with PCLeB.

- Thirty consecutive anterior STEMI patients were treated with PCLeB, a protocol involving intermittent reperfusion and lactated Ringer's solution injections.
- Immediate post-reperfusion coronary angiography was performed to assess for muscle squeezing.
- Follow-up angiography was conducted in 23 patients.
Main Results:
- Four out of 30 patients exhibited immediate left anterior descending artery (LAD) muscle squeezing post-reperfusion.
- An additional patient showed LAD muscle squeezing on follow-up angiography.
- Myocardial bridging was confirmed in 5 patients; 4 demonstrated immediate muscle squeezing.
Conclusions:
- Coronary artery muscle squeezing following reperfusion therapy is a notable finding in anterior STEMI patients undergoing PCLeB treatment.
- The phenomenon is not rare and may be associated with myocardial bridging.
