Background:

  • Left ventricular assist device (LVAD) therapy is increasingly used for advanced heart failure (HF).
  • Defining successful LVAD implantation remains challenging, impacting patient outcomes and therapy assessment.
  • Existing criteria may not fully capture the multifaceted success of LVAD support.

Purpose of the Study:

  • To develop and validate novel criteria for assessing successful left ventricular assist device (LVAD) implantation in heart failure (HF) patients.
  • To identify preoperative variables that predict successful LVAD outcomes.
  • To evaluate the proportion of patients achieving successful LVAD support based on the new definition.

Main Methods:

  • Retrospective analysis of 278 patients undergoing LVAD implantation between 2007 and 2015.
  • Definition of successful LVAD implant: alive or transplanted at 2 years, ≤2 readmissions in year 1, no major adverse events in year 1, and NYHA class ≤II at 6 months.
  • Univariate and multivariable analyses to identify predictors of LVAD success/failure.

Main Results:

  • A total of 278 patients received LVADs; 29% (81/278) met the defined success criteria.
  • Univariate analysis identified destination therapy indication, ischemic cardiomyopathy, and higher LVEF as predictors of failure.
  • Destination therapy indication was the only independent predictor of LVAD failure in multivariable analysis.

Conclusions:

  • The proposed criteria classified a minority of patients (29%) as successful LVAD implants.
  • Destination therapy indication is a significant independent predictor of LVAD failure.
  • Improving LVAD outcomes requires enhanced adverse event profiles, refined patient selection, and optimized implantation timing.
