Summary

Super-thick amniotic membrane grafts (ST-AMGs) provide a viable option for ocular surface reconstruction, demonstrating ease of suturing and good graft persistence. These grafts facilitated complete ocular surface epithelialization without initial symblepharon formation.

Background:

Purpose of the Study:

  • To evaluate the efficacy and safety of super-thick amniotic membrane grafts (ST-AMGs) for ocular surface reconstruction.
  • To assess intraoperative handling, graft stability, epithelialization, and complication rates associated with ST-AMGs.

Main Methods:

  • Retrospective, interventional case series involving surgical insertion of ST-AMGs for ocular surface reconstruction.
  • Eleven ST-AMGs were implanted following tumor resection (melanoma, squamous cell carcinoma, sebaceous carcinoma, atypical pterygium) and one for scleral necrosis.
  • Outcomes measured included graft handling, position, dissolution, epithelialization, symblepharon formation, visual acuity, intraocular pressure, and tumor control.

Main Results:

  • ST-AMGs, up to nine times thicker than standard grafts, were easily sutured without cheese-wiring.
  • Complete ocular surface epithelialization was achieved in all cases without primary symblepharon.
  • 83.3% of cases achieved complete local tumor control at a mean follow-up of 25.5 months, with one case requiring orbital exenteration.

Conclusions:

  • Super-thick amniotic membrane grafts (ST-AMGs) are technically easy to handle and suture, offering good persistence for ocular surface reconstruction.
  • ST-AMGs promote successful ocular surface epithelialization and are a valuable alternative for complex cases.
  • The study supports considering ST-AMGs as a reconstructive option for significant ocular surface defects.
