  • Retrospective monocentric study of 91 patients undergoing free flap surgery for chronic wounds (January 2001 - September 2016).
  • Evaluation of patient demographics, wound characteristics, flap types, and short- to long-term outcomes.

  • Free flap survival rate was 92.3% with a mean follow-up of 50 months.
  • Reconstructive failure occurred in 20.9% of cases.
  • Chronic osteomyelitis was the sole statistically significant factor predicting reconstruction failure (P=0.0169), increasing failure risk fivefold.

  • Free flaps demonstrate comparable reliability in treating chronic wounds to acute wound or post-oncological reconstruction.
  • Chronic osteomyelitis is a critical risk factor, elevating reconstruction failure risk by 500%.
  • Future research and evolving reconstruction paradigms will refine the role of muscle free flaps in chronic wound management.
