Extraosseous Ewing's sarcoma rarely occurs in the brain, often mimicking meningioma on imaging. Early recognition is crucial, especially in young patients with solitary dural-based lesions.

  • Extraosseous Ewing's sarcoma (EES) commonly affects the extremities, paravertebral, and retroperitoneal regions.
  • Primary intracranial EES is rare and typically presents as an intraparenchymal tumor when supratentorial.

Observation:

  • A case of a 20-year-old male presenting with neurological symptoms is described.
  • Radiographic imaging revealed an epidural lesion mimicking a meningioma.
  • Histopathological diagnosis confirmed the lesion as Ewing's sarcoma.

Findings:

  • Intracranial extraosseous Ewing's sarcoma can present as an extra-axial epidural tumor.
  • This rare presentation can radiographically mimic a meningioma.
