Summary

Posterior polar cataract surgery significantly reduces bothersome intraocular straylight, improving vision quality. Straylight measurement can guide surgical decisions even when visual acuity is preserved.

Background:

  • Posterior polar cataracts can cause significant visual disturbances.
  • Straylight, or light scatter, is a major factor affecting visual quality.

Purpose of the Study:

  • To evaluate intraocular straylight levels before and after surgical removal of posterior polar cataracts.

Main Methods:

  • A prospective case series involving 4 patients (8 eyes) with posterior polar cataracts.
  • Intraocular straylight was measured using the C-Quant straylight meter before and after cataract surgery.

Main Results:

  • Preoperative straylight levels were extremely high (2.01 log[s]), 13 times that of normal eyes.
  • Postoperative straylight significantly improved to 1.04 log(s) (P < .01).
  • Postoperative corrected distance visual acuity improved significantly from 0.15 logMAR to -0.08 logMAR (P < .01).

Conclusions:

  • Posterior polar cataracts cause substantial straylight, impacting vision quality despite relatively preserved visual acuity.
  • Cataract surgery effectively reduces straylight levels.
  • Straylight measurements can objectively quantify visual complaints and serve as an indication for surgery.
