  • To prospectively examine the association between youth perceptions of harm for various tobacco products and subsequent new use.
  • To investigate if lower harm perceptions predict initiation of non-cigarette tobacco products among adolescents.

Main Methods:

  • Utilized data from the Population Assessment of Tobacco and Health Study (2013-2015) with 10,081 US youth (12-17 years).
  • Assessed product-specific harm perceptions (cigarettes, e-cigarettes, cigars, pipes, hookah, smokeless tobacco) at Wave 1.
  • Calculated new product use at Wave 2 and used adjusted relative risks to determine predictive associations.

Main Results:

  • Cigarettes were perceived as most harmful (84.8%), while e-cigarettes were perceived as least harmful (26.6%).
  • Youth perceiving "no or little harm" from noncombustible products were more likely to initiate use.
  • Lower harm perceptions for e-cigarettes, hookah, and smokeless tobacco predicted subsequent trial of these products.

Conclusions:

  • Low perceived harm of non-cigarette tobacco products is a significant predictor of new use among youth.
  • Targeting specific harm perceptions related to non-cigarette tobacco products may be an effective prevention strategy.
  • Interventions should address the underestimation of harm associated with newer tobacco products to curb youth uptake.
