Anticoagulation-related nephropathy (ARN) can occur with novel oral anticoagulants. This case study presents ARN in a patient receiving only dual antiplatelet therapy, challenging current understanding.

Background:

  • Anticoagulation-related nephropathy (ARN) is a kidney complication associated with excessive anticoagulation, typically warfarin (INR > 3.0).
  • Diagnosis involves renal biopsy showing acute tubular necrosis and red blood cell casts.
  • ARN has also been reported with novel oral anticoagulants.

Observation:

  • A case of an 82-year-old male patient is presented.
  • The patient developed biopsy-proven ARN.
  • He had received dual antiplatelet therapy without prior anticoagulation and had normal coagulation tests.

Findings:

  • This case suggests ARN can occur with dual antiplatelet therapy alone.
  • It challenges the established link between ARN and anticoagulation.
