This case study reports acral vasculitis in a patient treated with anti-CTLA-4 and anti-PD-L1 immunotherapy for metastatic bladder cancer. Oral corticosteroids effectively improved symptoms, contrasting with previous findings.
Area of Science:
- Immunology
- Oncology
- Dermatology
Background:
- Immune checkpoint inhibitors (ICIs) like anti-CTLA-4 and anti-PD-L1 are used in cancer therapy.
- Vasculitis is a known, albeit rare, side effect of ICI treatment.
- Previous reports documented ipilimumab-induced vasculitis in melanoma patients.
Purpose of the Study:
- To report a case of acral vasculitis associated with a combination of anti-CTLA-4 (tremelimumab) and anti-PD-L1 (durvalumab).
- To compare the clinical presentation and treatment response with previously reported cases of ICI-induced vasculitis.
Main Methods:
- Case report of a patient with metastatic urothelial bladder cancer.
- Treatment involved a combination regimen of tremelimumab and durvalumab.
- Clinical observation and assessment of treatment response to corticosteroids.
Main Results:
- The patient developed acral vasculitis during combination ICI therapy.
- Unlike previous reports, this case showed significant improvement with oral corticosteroids.
- This suggests a potentially different management approach for this specific ICI-induced vasculitis.
Conclusions:
- Combination anti-CTLA-4 and anti-PD-L1 therapy can induce acral vasculitis.
- Oral corticosteroids may be an effective treatment for this adverse event.
- Further research is needed to understand ICI-induced vasculitis mechanisms and optimal management strategies.
