Summary

Gingival fibroblasts secrete factors that inhibit bone formation by blocking bone morphogenetic proteins (BMPs). These inhibitors, especially Gremlin1, may hinder bone regeneration in dental and periodontal surgery.

Background:

  • Superficial gingival connective tissues may impede alveolar bone regeneration.
  • Gingival fibroblasts (GF) might inhibit bone morphogenetic protein (BMP)-induced osteoblastic differentiation.

Purpose of the Study:

  • Investigate GF inhibition of BMP-induced osteoblastic differentiation.
  • Determine GF expression of BMP inhibitors.
  • Assess if reducing inhibitors can restore osteoblastic differentiation.

Main Methods:

  • Co-culture of GF and osteoblasts to assess alkaline phosphatase (ALP) activity.
  • Stimulation of osteoblastic cells with BMP2 and GF-conditioned media (GFCM).
  • Analysis of BMP inhibitor gene expression (RT-qPCR, Western blot, in situ hybridization) and removal via immunoprecipitation.

Main Results:

  • GF-secreted factors inhibited BMP-stimulated ALP activity.
  • GFCM blocked BMP activity comparably to Noggin.
  • GF constitutively expressed BMP antagonists (Grem1, Grem2, Nbl1) and Wnt inhibitor (Sfrp1), with Gremlin1 showing highest basal expression.
  • Gremlin1 removal from GFCM abolished inhibition; its re-addition restored it.

Conclusions:

  • Gingival fibroblasts secrete BMP inhibitors, including Gremlin1.
  • These inhibitors suppress BMP-induced bone formation.
  • Gremlin1 and other inhibitors may limit coronal alveolar bone regeneration in oral surgery.
