Methemoglobinemia, a condition causing cyanosis, was treated with hyperbaric oxygen therapy (HBO) when standard treatments failed. This alternative therapy effectively resolved symptoms in a patient exposed to paint thinner and zinc phosphate.
Area of Science:
- Toxicology
- Emergency Medicine
- Hematology
Background:
- Methemoglobinemia is a rare condition causing cyanosis, often managed with oxygen and methylene blue.
- Exposure to certain chemicals, like paint thinner and zinc phosphate, can induce methemoglobinemia.
- Refractory cases necessitate exploration of alternative treatment strategies.
Observation:
- A 52-year-old male presented with severe cyanosis and dyspnea after chemical exposure.
- Arterial blood gas revealed a MetHb level of 49.1mm/Hg, indicating severe methemoglobinemia.
- The patient was unresponsive to conventional high-flow oxygen and methylene blue therapies.
Findings:
- Hyperbaric oxygen therapy (HBO) was administered as an alternative treatment.
- The patient experienced prompt symptom resolution and MetHb level normalization following HBO.
- This case highlights HBO as a viable treatment for refractory methemoglobinemia.
Implications:
- Hyperbaric oxygen therapy (HBO) may be a crucial alternative for methemoglobinemia cases resistant to standard treatments.
- This case expands treatment options for persistent hypoxia and cyanosis in methemoglobinemia.
- Further research into HBO's efficacy for methemoglobinemia is warranted.
