Area of Science:

  • Mycology
  • Infectious Diseases
  • Hematology

Background:

  • Mucormycosis is a rare but aggressive invasive fungal infection.
  • It predominantly affects immunocompromised individuals, including transplant recipients.
  • Cutaneous mucormycosis presents a diagnostic and therapeutic challenge.

Observation:

  • A 68-year-old patient with acute myeloid leukemia underwent allogeneic hematopoietic stem cell transplantation.
  • The patient developed invasive pulmonary aspergillosis, diabetes mellitus, and subsequently, cutaneous mucormycosis due to Rhizopus pusillus.
  • Despite treatment with liposomal amphotericin B and posaconazole, the patient experienced septic shock and multi-organ failure.

Findings:

  • This case illustrates a fatal outcome of cutaneous mucormycosis in an immunocompromised patient post-hematopoietic stem cell transplantation.

  • Rhizopus pusillus was identified as the causative agent of the mucormycosis.
  • The patient's complex medical history, including graft-versus-host disease and glucocorticoid therapy, likely contributed to the infection's severity.
  • Implications:

    • Early recognition and aggressive management are crucial for improving outcomes in transplant patients with mucormycosis.
    • Understanding risk factors and clinical presentations in immunocompromised populations is vital for timely diagnosis.
    • Further research into optimal treatment strategies for cutaneous mucormycosis in transplant recipients is warranted.
