Elevated platelet-to-lymphocyte ratio (PLR) is a significant predictor of post-stroke depression (PSD). Stratifying PLR levels can enhance the prediction of PSD in acute ischemic stroke patients.

- The highest PLR tertile showed a 5.154-fold increased odds of PSD compared to the lowest tertile, independent of other factors.

- Increased PLR at admission is a significant independent biomarker for predicting PSD.
- Stratified PLR analysis enhances predictive power for PSD development.
