Background:

  • Acute cardiogenic pulmonary edema (ACPE) is a critical condition requiring prompt diagnosis.

Purpose of the Study:

  • To conduct a comprehensive ultrasound evaluation of patients diagnosed with acute cardiogenic pulmonary edema.
  • To assess left and right ventricular function, heart dimensions, pulmonary pressures, and valvular disease in ACPE patients.

Main Methods:

  • A registry of 70 ACPE patients was established from two emergency hospitals.
  • Transthoracic ultrasound was performed within the first three days of admission.
  • Evaluated parameters included left ventricular systolic/diastolic function, right ventricular function, cardiac dimensions, pulmonary artery pressure, and valvular disease severity.

Main Results:

  • Mean left ventricular ejection fraction was 39.5±11.4%, indicating systolic dysfunction.
  • All evaluable patients exhibited diastolic dysfunction.
  • Mean pulmonary artery pressure was elevated at 40.9±16.7 mmHg.
  • Mitral and tricuspid insufficiency were prevalent, with moderate to severe impairment in 82.8% and 65.7% of patients, respectively.

Conclusions:

  • Ultrasound effectively identifies significant cardiac abnormalities in ACPE.
  • Findings highlight prevalent systolic and diastolic dysfunction, elevated pulmonary pressures, and valvular disease in ACPE.
  • Echocardiography is crucial for guiding management strategies in ACPE.
