Background:

  • Secondary bone marrow involvement is common in non-Hodgkin's lymphoma (NHL).
  • Primary isolated bone marrow involvement (PBML) in NHL is exceptionally rare, especially with long-term complete remission (CR).

Observation:

  • A patient presented with primary bone marrow diffuse large B cell lymphoma (PBML/DLBCL).
  • Symptoms included prolonged high-grade fever and systemic purpura due to severe thrombocytopenia.

Findings:

  • The patient received R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisolone) chemotherapy.
  • Achieved and maintained complete remission (CR) for 8 years post-diagnosis without relapse.

Implications:

  • This case demonstrates the possibility of long-term, durable remission in rare primary bone marrow DLBCL.
  • Highlights the efficacy of R-CHOP in achieving sustained CR for this rare presentation.
  • Contributes to the limited literature on successful management of PBML/DLBCL.
