Calculous cholecystitis is more common than acalculous cholecystitis in immunocompromised patients. Both conditions are successfully treated with laparoscopic cholecystectomy, showing low complication rates.
Area of Science:
- Gastroenterology
- Immunology
- Surgical Oncology
Background:
- Acute cholecystitis poses a significant threat to immunocompromised individuals.
- Understanding its incidence, clinical course, and management in this population is crucial.
Purpose of the Study:
- To determine the incidence of acute cholecystitis in immunocompromised patients.
- To analyze the clinical course and management strategies for both calculous and acalculous forms of the disease in this group.
Main Methods:
- A single-center database was queried for acute cholecystitis cases between 2003 and 2016.
- Patients with concomitant neutropenia, leukopenia, leukemia, or lymphoma were identified.
- Data on demographics, diagnostics, and management were collected and analyzed.
Main Results:
- Out of 4525 acute cholecystitis patients, 120 were immunocompromised.
- Calculous acute cholecystitis occurred in 65.8% of cases, while acalculous occurred in 34.2%.
- Laparoscopic cholecystectomy was the primary management for both types, with similar low complication rates.
Conclusions:
- Contrary to common belief, calculous cholecystitis is more prevalent than acalculous cholecystitis in immunocompromised patients.
- Laparoscopic cholecystectomy is an effective and safe treatment for both forms of acute cholecystitis in this vulnerable population.
- Low conversion rates to open surgery were observed.
