Summary

Cephalosporin allergy is often not a class effect. Most patients allergic to one cephalosporin are only allergic to that specific drug, not other cephalosporins or penicillins.

Background:

  • Patients with IgE-mediated allergy to cephalosporins are often presumed to have cross-reactivity to other cephalosporins and penicillins.
  • This assumption can lead to unnecessary avoidance of essential antibiotics.

Purpose of the Study:

  • To investigate cross-reactivity patterns in patients with confirmed cephalosporin allergy.
  • To clarify the risk of allergy to other beta-lactam antibiotics.

Main Methods:

  • Investigated patients with a history of immediate allergy to cephalosporins using specific IgE testing, skin prick tests, intradermal tests, and drug provocation tests.
  • Tested for allergies to penicillin, amoxicillin, cefaclor, and a panel of other penicillins and cephalosporins.

Main Results:

  • Out of 90 patients with cephalosporin allergy history, 24 were confirmed allergic to their index cephalosporin.
  • Most (83.3%) of these 24 patients were only allergic to their index cephalosporin.
  • Co-sensitization to other cephalosporins or penicillins was uncommon and often linked to similar side chains.

Conclusions:

  • Cephalosporin allergy is typically not a class effect.
  • Cross-reactivity to other cephalosporins and penicillins is infrequent in patients with confirmed cephalosporin allergy.
  • Allergy testing can help identify specific sensitivities and guide antibiotic selection.
