Background:

  • The visual pathway is a sensitive indicator of CNS integrity.

Purpose of the Study:

  • To assess the functional integrity of the visual pathway in hypothyroid patients using pattern visual evoked potential (PVEP).

Main Methods:

  • 36 female patients with hypothyroidism (overt and subclinical) and 36 healthy controls underwent ophthalmic examination and PVEP testing.
  • PVEP was recorded using check sizes subtending 15 and 60 minutes of arc.

Main Results:

  • For 15-minute arc check size, PVEP showed significantly delayed P100 latency and reduced amplitude in hypothyroid patients compared to controls.
  • Overt hypothyroidism was associated with more pronounced delays and amplitude reductions.
  • No significant differences were observed with the 60-minute arc check size.
  • A correlation was found between disease severity and PVEP components.

Conclusions:

  • Reduced thyroid hormone levels are linked to visual pathway dysfunction, specifically delayed PVEP responses to small visual stimuli.
  • The severity and duration of hypothyroidism correlate with the degree of CNS involvement affecting the visual pathway.
