A complex heart failure case involving ventricular thrombus was successfully treated with ventricular reconstruction and HeartMate 3™ Left Ventricular Assist Device (LVAD) implantation. This combined approach offers a viable solution for patients with challenging cardiac conditions.

- The patient experienced an uneventful recovery and was discharged on postoperative day 14.

- This case demonstrates the feasibility of combined ventricular reconstruction and LVAD implantation in complex heart failure patients with ventricular thrombus.
- The surgical strategy offers a potential solution for improving outcomes in challenging LVAD cases.
