Summary

Spontaneous remission of aggressive diffuse large B-cell lymphoma is rare but possible, particularly in elderly patients after biopsy. This suggests enhanced T-cell immunity may drive lymphoma regression.

Background:

  • Spontaneous remission (SR) in solid tumors is known, but exceedingly rare in aggressive diffuse large B-cell lymphoma (DLBCL).
  • Previous reports suggest complex tumor-intrinsic and microenvironment factors contribute to SR.
  • Understanding SR mechanisms in DLBCL is crucial for novel therapeutic strategies.

Observation:

  • A case of an 88-year-old woman with transformed follicular lymphoma (a type of DLBCL) achieving morphologic spontaneous remission 3 months post-biopsy.
  • Analysis of 16 similar DLBCL cases indicated SR is more frequent in elderly patients shortly after biopsy.
  • SR was also associated with Epstein-Barr virus-driven and activated B-cell type lymphomas.

Findings:

  • Anti-tumor T-cell responses appear augmented in a subset of patients experiencing lymphoma regression.
  • Tissue biopsy may enhance pre-existing, "primed" innate anti-tumor T-cell immunity in immunogenic lymphoma subtypes.
  • The study reinforces the role of innate T-cell anticancer immunity in lymphoma regression.

Implications:

  • The findings suggest a potential proof-of-concept for investigating novel pathways to enhance T-cell therapy.
  • This could lead to improved treatment outcomes for patients with aggressive lymphomas.
  • Further research into biopsy-induced immune responses may uncover new therapeutic targets.
