Summary

Normal epithelial lining with corrupted shapes (NECS) are found beneath non-polypoid colonic adenomas. This study assessed NECS frequency in flat adenomas (FAs) and lateral-spreading adenomas (LSAs), finding their presence significant in adenoma development.

Background:

  • Previously, normal epithelial lining with corrupted shapes (NECS) were identified beneath polypoid adenomas in rats and humans.
  • NECS represent a distinct histological feature observed in the colonic mucosa.

Purpose of the Study:

  • To investigate the prevalence of NECS in non-polypoid colonic adenomas, specifically flat adenomas (FAs) and lateral-spreading adenomas (LSAs).
  • To determine if NECS contribute to the histogenesis of non-polypoid colorectal adenomas.

Main Methods:

  • Histological analysis of 51 non-polypoid colonic adenomas (27 FAs, 24 LSAs) using hematoxylin and eosin staining.
  • Evaluation of NECS within a 10-mm field of vision (FOV) and calculation of the NECS/mm ratio for each lesion.
  • Distinguishing FAs as lesions within the FOV and LSAs as those exceeding the FOV limit.

Main Results:

  • All 51 non-polypoid adenomas examined were identified as tubular adenomas.
  • A significantly higher mean number of NECS was observed in LSAs (15.29) compared to FAs (6.29) (p<0.05).
  • No significant difference in the mean NECS/mm ratio was found between FAs (0.94) and LSAs (0.92).

Conclusions:

  • The presence of NECS beneath the neoplastic epithelium of FAs and LSAs is notable, contrasting with their rarity in normal colonic mucosa.
  • The findings suggest that NECS may play a significant role in the histogenesis of non-polypoid colonic adenomas.
  • This previously unaddressed phenomenon warrants further investigation into its implications for colorectal cancer development.
