Background:

  • Three-dimensional (3D), high-definition (HD) endoscopy is a recent advancement in neurosurgery.
  • Its application in endonasal skull base surgery is well-documented, but clinical data on 3D-HD ventriculoscopes is limited.

Purpose of the Study:

  • To describe the initial clinical experience with a novel 3D-HD ventriculoscope in neurosurgery.
  • To evaluate the feasibility, safety, and preliminary outcomes of using this new device.

Main Methods:

  • A prospective collection of patients undergoing surgery with a 3D-HD ventriculoscope from June 2016 to June 2018.
  • The study involved 24 patients (age 3-84 years) undergoing 25 procedures, including endoscopic third ventriculocisternostomy, biopsy, and cyst fenestration.
  • Data collected included demographics, symptoms, neurological status, imaging, surgical details, complications, and follow-up.

Main Results:

  • The technical goals of all 25 procedures were successfully achieved.
  • No intraoperative complications occurred, except for one epileptic seizure.
  • Postoperative complications were infrequent, including asymptomatic subdural collections (2 patients), infection (1 patient), and delayed endoscopic third ventriculocisternostomy closure (1 patient).
  • Image quality was satisfactory, although limitations included the device size and the 0-degree optic.

Conclusions:

  • The 3D-HD ventriculoscope shows potential advantages for ventricular surgery.
  • This initial experience is promising and warrants further investigation with larger patient cohorts.
