Colchicine may reduce stroke and TIA incidence, but current evidence is limited. More large-scale trials are needed to confirm the protective benefits of colchicine for stroke prevention.
Area of Science:
- Cardiovascular Medicine
- Pharmacology
Background:
- Inflammation plays a key role in atherothrombosis.
- Colchicine, an anti-inflammatory agent, is being investigated for cardiovascular benefits.
Purpose of the Study:
- To systematically review the efficacy of colchicine in preventing primary or secondary stroke/transient ischemic attack (TIA).
Main Methods:
- Searched MEDLINE, EMBASE, CENTRAL, and OpenGrey for studies on colchicine and stroke/TIA.
- Included case-control, cohort studies, and RCTs reporting stroke/TIA incidence.
- Pooled data using relative risk ratios (RRs) with 95% CIs.
Main Results:
- Five studies (4 RCTs, 1 cohort) with 2170 patients were included.
- Overall, colchicine use was associated with lower stroke/TIA incidence (RR, 0.37; P=.0002).
- However, RCTs alone showed no significant effect (RR, 0.61; P=.57), while the cohort study suggested a benefit (RR, 0.33; P=.0002).
Conclusions:
- Colchicine shows potential for preventing stroke/TIA, but current data are inconclusive.
- Small sample sizes in existing RCTs limit definitive conclusions.
- Large-scale pragmatic RCTs are necessary to establish colchicine's role in stroke prevention.
