    Area of Science:

    • Nephrology
    • Endocrinology
    • Rheumatology

    Background:

    • Phosphate diabetes is traditionally considered a rare condition primarily affecting children.
    • Adult-onset phosphate diabetes is increasingly recognized, particularly in individuals with underlying rheumatic or kidney conditions.

    Observation:

    • This study examined 24 adult patients diagnosed with hypophosphataemia and hyperphosphaturia over a six-month period.
    • Patients presented with diverse diagnoses including myalgia, dorsalgia, prostatitis, and various kidney diseases, alongside asthenia.

    Findings:

    • Mean serum phosphorus was 0.7 mmol/l, with mean phosphate clearance of 31 ml/min/1.73 m2.
    • Common symptoms included bone pain, tiredness, and dizziness.
    • Laboratory findings revealed high urine pH, hyperaminoaciduria, and phosphate crystals in dried urine.

    Implications:

    • The findings suggest that hypophosphataemia and hyperphosphaturia are more prevalent in adults than previously assumed.
    • Early identification of phosphate diabetes in adults with rheumatic or kidney diseases is crucial for timely management.
    • Further research is warranted to elucidate the specific mechanisms and long-term consequences of adult-onset phosphate diabetes.
