Human experts show good agreement for Diffusion-Weighted Imaging Alberta Stroke Program Early CT Score (DWI-ASPECTS). Automated methods are slightly less precise but clinically viable for stroke assessment.
Area of Science:
- Neurology
- Radiology
- Medical Imaging
Background:
- The Alberta Stroke Program Early CT Score (ASPECTS) is crucial for evaluating early ischemic changes in stroke.
- Diffusion-weighted MRI (DWI) is a sensitive technique for detecting acute ischemic stroke.
Purpose of the Study:
- To evaluate the intra- and inter-rater reliability of DWI-ASPECTS.
- To compare human expert DWI-ASPECTS scoring with automated methods (eASPECTS).
Main Methods:
- Two experts rated DWI scans from 96 acute ischemic stroke patients.
- Automated methods included atlas-based thresholding and a regression tree learning method.
- Agreement was assessed using intraclass correlation coefficients (ICC) and Bland Altman plots.
Main Results:
- High intra-rater agreement was observed (ICC 0.91-0.93).
- Inter-rater agreement was good (ICC 0.86), with a slight bias.
- Agreement between human raters and automated methods ranged from 0.81 to 0.87.
Conclusions:
- DWI-ASPECTS agreement is comparable or superior to CT-based ASPECTS.
- Automated methods show slightly lower performance than human experts but offer good precision for clinical use.
