The RPE-RPE fit optical coherence tomography angiography (OCTA) slab provides the best agreement for assessing neovascularization (NV) in age-related macular degeneration (AMD). This method offers higher reproducibility and larger measurements for type 1 NV lesions.
Area of Science:
- Ophthalmology
- Medical Imaging
- Retinal Diseases
Background:
- Age-related macular degeneration (AMD) is a leading cause of vision loss.
- Neovascularization (NV) is a key feature of AMD, driving disease progression.
- Optical coherence tomography angiography (OCTA) is a crucial imaging modality for NV assessment.
Purpose of the Study:
- To systematically compare intermodality and interreader agreement for AMD-associated NV assessment using OCTA images from different slabs.
- To evaluate the reproducibility of NV measurements across various OCTA image reconstructions.
Main Methods:
- Data from 48 patients (50 eyes) with AMD and type 1 or 2 NV were analyzed.
- Swept source (SS)-OCTA was used to acquire images.
- Three OCTA en face image slabs were generated: outer retina to choriocapillaris (ORCC), choriocapillaris (CC), and retinal pigment epithelium (RPE)-RPE fit.
- Two readers assessed interreader variability; single images assessed intermodality variability.
Main Results:
- The RPE-RPE fit slab demonstrated the highest interreader reproducibility for type 1 NV assessment (lowest mean absolute difference: 0.16 mm²).
- Intraclass correlation coefficient (ICC) and coefficient of repeatability (CR) confirmed superior reproducibility with the RPE-RPE fit slab.
- Type 1 NV measurements were significantly larger on RPE-RPE fit images (0.62 mm²) compared to CC images (0.00 mm²) and comparable to ORCC images (0.58 mm²).
Conclusions:
- Retinal pigment epithelium (RPE)-RPE fit OCTA images offer the highest interreader agreement for neovascularization assessment in AMD.
- This imaging slab provides more accurate and reproducible measurements of type 1 NV lesions.
- OCTA imaging holds potential for use in clinical trials evaluating novel treatments for NV.
