Summary

Thromboelastometry is not sensitive enough for diagnosing mild bleeding disorders (MBDs), the most common type. This study found no significant differences in test results between patients with and without MBDs, questioning its diagnostic utility.

Purpose of the Study:

  • To evaluate the sensitivity of thromboelastometry in diagnosing MBDs.
  • To determine if thromboelastometry can differentiate MBD patients from healthy individuals.

Main Methods:

  • A prospective cohort study included 217 patients with suspected bleeding disorders.
  • Thromboelastometry was performed using ROTEM delta (EXTEM, INTEM, FIBTEM).
  • Diagnosis of MBD was confirmed using current guidelines and the ISTH bleeding assessment tool.

Main Results:

  • Mild bleeding disorders were diagnosed in 45% of patients.
  • No significant differences in thromboelastometry variables (clotting time, maximum clot firmness) were observed between patients with and without MBDs.
  • Most thromboelastometry results fell within established reference ranges, irrespective of MBD status.

  • Thromboelastometry demonstrated insufficient sensitivity for diagnosing mild bleeding disorders.
  • Current viscoelastic methods may not be suitable as a primary diagnostic tool for MBDs.
