Vedolizumab for chronic antibiotic-refractory pouchitis

Vedolizumab shows promise for treating chronic antibiotic-refractory pouchitis (CARP) after ileal pouch-anal anastomosis (IPAA). This gut-selective therapy improved symptoms and endoscopic scores in most patients with a favorable safety profile.

Background:
- Chronic antibiotic-refractory pouchitis (CARP) is a challenging complication following ileal pouch-anal anastomosis (IPAA).
- Limited data exist on the efficacy of vedolizumab, a gut-selective anti-integrin antibody, for CARP treatment.
Purpose of the Study:
- To assess the efficacy and safety of vedolizumab as induction therapy for patients with CARP.

- A single-center, historic cohort study analyzed 19 CARP patients treated with vedolizumab from January 2015 to June 2017.
- Inclusion criteria included active pouchitis (modified pouch disease activity index [mPDAI] score ≥5) or clinician diagnosis.
- mPDAI scores were calculated using pouchoscopy and clinical visits pre-treatment and at 3-month post-therapy.
Main Results:
- 19 patients (mean age 26.7 years, 53% male) were included.
- 74% of patients showed improvement in endoscopic and total mPDAI scores (median change -2 units, P=0.031).
- Adverse events occurred in 11% of patients, and 21% required surgery for CARP.
Conclusions:
- Vedolizumab appears to be an effective and safe treatment option for CARP.
- Larger studies are needed to confirm these findings and establish vedolizumab's role in CARP management.
