Accelerated partial breast irradiation (APBI) using intensity-modulated radiation therapy (IMRT) demonstrated excellent 1-year cosmetic outcomes and minimal toxicity in early-stage breast cancer patients. The study found no grade 2 fibrosis, supporting APBI as a safe and effective treatment option.
Area of Science:
- Oncology
- Radiation Oncology
- Breast Cancer Treatment
Background:
- Accelerated partial breast irradiation (APBI) is an evolving treatment for early-stage breast cancer.
- Intensity-modulated radiation therapy (IMRT) offers precise dose delivery for APBI.
Purpose of the Study:
- To evaluate 1-year cosmetic results and toxicity of APBI using IMRT.
- To assess outcomes of a prospective phase II trial delivering 27 Gy in 5 fractions.
Main Methods:
- Prospective phase II trial involving node-negative breast cancer patients post-surgery.
- IMRT delivered 27 Gy in 5 daily fractions.
- Cosmetic outcomes assessed via clinical photos, EORTC scores, and physician panel consensus at baseline and 1-year.
Main Results:
- 55 patients completed 1-year follow-up; 53% showed cosmetic improvement, 40% no change.
- Only 4% of patients with excellent/good baseline scores experienced cosmetic deterioration.
- No telangiectasia or grade 2+ fibrosis observed; no recurrences.
Conclusions:
- APBI with 27 Gy in 5 fractions yields acceptable 1-year cosmesis and no significant toxicity.
- The observed low rate of fibrosis supports the safety of this APBI regimen.
- The trial continues to accrual target of 274 patients.
