Abstract:
From what initial assessment of renal function seek advice from the nephrologist? The discovery of chronic kidney disease immediately raises the issue of care coordination between the primary care physician and the nephrologist. The role of the nephrologist is to establish the diagnosis of the primary renal disease, to set up its specific treatment, to evaluate renal and global risk, to propose a personalized care program, to provide therapeutic education, and to prepare the patient to replacement therapies in the presence of an advanced or rapidly evolving chronic kidney disease. Depending on the nature of the kidney disease, its stage, its complications, but also the availability of care resources, sharing tasks between caregivers shall adapt over time to fulfil the expectations of the informed patient.
