Background:

  • Ankylosing spondylitis (AS) is a chronic inflammatory disease primarily affecting the axial skeleton.
  • Sensorineural deafness is documented in AS, but conductive hearing loss (HL) is considered rare.
  • The prevalence and characteristics of HL in AS patients require further investigation.

Purpose of the Study:

  • To determine the prevalence of hearing loss (HL) in ankylosing spondylitis (AS) patients.
  • To investigate the association between HL and non-steroidal anti-inflammatory drug (NSAID) intake in AS.
  • To characterize the type and frequency distribution of hearing loss in AS.

Main Methods:

  • A study cohort of 100 AS patients and 40 controls was evaluated.
  • Hearing loss was defined as pure-tone thresholds exceeding 20 dB at two or more audiogram frequencies.
  • Clinical data, including cumulative NSAID doses, were collected and analyzed.

Main Results:

  • Hearing loss was present in 48% of AS patients, with conductive HL being more common than sensorineural HL.
  • Mild HL was observed in 38 patients, while 10 had moderate to severe HL.
  • HL was associated with increased age but not with disease activity indices (BASDAI, BASFI, BASMI) or cumulative NSAID dose.

Conclusions:

  • Hearing loss is a common comorbidity in ankylosing spondylitis.
  • Conductive hearing loss is more frequent than sensorineural HL in AS patients.
  • The observed HL is typically mild, affects low frequencies, and is not linked to NSAID usage.
