A new colistin-resistant Acinetobacter baumannii strain was identified in a Colombian patient. This finding highlights emerging resistance to critical antibiotics like colistin in Acinetobacter baumannii infections.
Area of Science:
- Infectious Diseases
- Microbiology
- Antimicrobial Resistance
Background:
- Carbapenem-resistant Acinetobacter baumannii (CRAB) poses a significant global health threat.
- Polymyxins (colistin and polymyxin B) are last-resort antibiotics for CRAB infections.
- Emerging resistance to colistin in CRAB necessitates urgent investigation.
Observation:
- A case of bacteremia caused by a colistin- and carbapenem-resistant A. baumannii isolate is presented.
- The patient was a 64-year-old male admitted to an intensive care unit in Colombia.
- Clinical and molecular typing was performed on the A. baumannii isolate.
Findings:
- The identified A. baumannii strain exhibited resistance to both colistin and carbapenems.
- Novel mechanisms of colistin resistance, not previously reported, were identified in this isolate.
- The patient's bacteremia responded adequately to a multi-drug antibiotic regimen.
Implications:
- This case underscores the critical need for continuous surveillance of antimicrobial resistance.
- Understanding novel resistance mechanisms is crucial for developing effective treatment strategies.
- The findings emphasize the importance of preserving the efficacy of last-resort antibiotics like colistin.
