Summary

High stroke severity (NIHSS >4) and prolonged status epilepticus (>16 hours) predict post-stroke epilepsy (PSE) in patients with early-onset seizures. These factors may also indicate earlier seizure recurrence after stroke.

Background:

  • Post-stroke epilepsy (PSE) is a common complication following stroke.

Purpose of the Study:

  • To evaluate the incidence of post-stroke epilepsy (PSE) in patients experiencing early-onset post-stroke status epilepticus (PSSE).
  • To identify factors associated with the development of PSE after early-onset PSSE.

Main Methods:

  • A cohort of 50 patients with early-onset PSSE and no prior epilepsy history was analyzed.
  • Factors including stroke severity (NIHSS) and duration of status epilepticus were assessed.
  • Multivariate analysis was employed to determine independent predictors of PSE.

Main Results:

  • Twenty percent of patients developed PSE, with a median delay of 153 days.
  • Higher NIHSS score (>4) and longer PSSE duration (>16 hours) were independently associated with an increased risk of PSE.

  • These findings aid in identifying high-risk individuals for closer monitoring and potential prophylactic interventions.
  • Longer PSSE duration may correlate with earlier onset of recurrent seizures post-stroke.
