Implementing an opioid-free headache and migraine treatment in emergency departments (EDs) reduced patient revisits. This approach also decreased hospital admissions and ED length of stay compared to opioid use.
Area of Science:
- Emergency Medicine
- Neurology
- Public Health
Background:
- The opioid epidemic strains emergency department (ED) resources.
- Headache and migraine treatments in the ED are a significant concern.
Purpose of the Study:
- To assess the impact of an opioid-free headache and migraine treatment algorithm on patient-centered outcomes in the ED.
- To evaluate the effectiveness of opioid-free protocols in managing headache and migraine patients.
Main Methods:
- Retrospective review of 2953 patient encounters for headache or migraine in EDs.
- Comparison of patient outcomes before and after implementing an opioid-free treatment algorithm.
- Analysis of patients treated with or without opioids, focusing on 30-day revisit rates.
Main Results:
- Opioid-free algorithm implementation correlated with a lower 30-day ED revisit rate (6.3% vs. 8.2%).
- Patients treated with opioids showed higher 30-day revisit rates (15.2% vs. 6.3%), increased admissions, and longer ED stays.
- Reduced revisit incidence was observed within the entire study period (9.2% vs. 12.1%) post-algorithm.
Conclusions:
- Opioid use for ED headache and migraine treatment is linked to negative patient outcomes.
- Opioid-free strategies may improve patient outcomes, reduce hospital admissions, and decrease ED length of stay.
- Adopting opioid-free algorithms is a viable approach to mitigate risks associated with opioid use in headache and migraine management.
