Hemodialysis I: Introduction01:25

Hemodialysis I: Introduction

1.5K
Hemodialysis (HD) is a medical treatment that artificially removes waste products, excess fluids, and toxins from the blood when the kidneys are no longer able to perform these functions effectively. In this process, blood is filtered through a semipermeable membrane, allowing for the selective removal of waste while preserving necessary components like blood cells and proteins. Hemodialysis is typically performed in patients with end-stage renal disease (ESRD) or severe kidney...

Hemodialysis II: Procedure and Complications

DialyzersA hemodialysis (HD) dialyzer is a plastic cartridge containing thousands of parallel hollow fibers, which serve as semipermeable membranes. These fibers are typically made from cellulose-based or other synthetic materials. During HD, blood is pumped into the top of the cartridge and distributed among these fibers. Simultaneously, dialysis fluid, known as dialysate, is introduced into the bottom of the cartridge, bathing the outside of the fibers. Across the semipermeable membrane,...

Instrument Transformers

457
Instrument transformers, comprising voltage transformers (VTs) and current transformers (CTs), play crucial roles in power substations by providing isolated replicas of current or voltage for measurement and protection purposes. Voltage transformers reduce the primary voltage to levels suitable for relay operation and measurement, while current transformers scale down the primary current. The primary winding of a current transformer often consists of a single turn, achieved by threading the...

Instrument Calibration

695
Instrument calibration is essential for ensuring that instruments produce accurate and consistent results. It is vital in manufacturing, healthcare, testing laboratories, and scientific research. Calibration processes are specific to each instrument and help enhance data accuracy. Each instrument has a unique calibration process tailored to its design and function to improve data accuracy.

Instrumentation Amplifier

An electrocardiography (ECG) machine is an essential piece of medical equipment used to monitor the electrical activity of the heart. It operates by detecting small electrical changes on the skin that result from the depolarization of the heart muscle during each heartbeat. However, these signals are in the microvolt range and can be easily overwhelmed by noise or interference.
To overcome this challenge, an ECG machine utilizes an instrumentation amplifier. This specialized amplifier is...

Uncertainty in Measurement: Reading Instruments

50.9K
Counting is the type of measurement that is free from uncertainty, provided the number of objects being counted does not change during the process. Such measurements result in exact numbers. By counting the eggs in a carton, for instance, one can determine exactly how many eggs are there in the carton. Similarly, the numbers of defined quantities are also exact. For example, 1 foot is exactly 12 inches, 1 inch is exactly 2.54 centimeters, and 1 gram is exactly 0.001 kilograms. Quantities...

Summary

Patients on hemodialysis (HD) in developing countries show good functional status (FS) based on FIM and ADLQ scales. However, mobility and motor tasks are more affected, with younger patients (<40) exhibiting poorer FS.

Area of Science:

  • Nephrology
  • Gerontology
  • Rehabilitation Medicine

Background:

  • Limited data exists on functional status (FS) for hemodialysis (HD) patients in developing nations.
  • Chronic kidney disease (CKD) etiology and demographics differ significantly in these regions.
  • Assessing FS is crucial for understanding patient well-being and care needs.

Purpose of the Study:

  • To evaluate the functional status of patients undergoing hemodialysis (HD) in a developing country.
  • To compare the efficacy of three validated generic instruments in assessing FS.
  • To identify specific functional domains most affected in this patient population.

Main Methods:

  • A cross-sectional study involving 116 adult HD patients with >3 months dialysis vintage.
  • Functional status assessed using Functional Independence Measure (FIM), Seven domains and five levels (7D5L), and Activities of Daily Living Questionnaire (ADLQ).
  • Data analysis focused on mean scores and proportion of patients demonstrating independence across instruments.

Main Results:

  • Patients generally showed good functional independence via FIM and ADLQ, with over 80% independence on ADLQ.
  • The 7D5L scale identified more limitations, suggesting sensitivity to mild-moderate functional deficits.
  • Locomotion, mobility, and motor task domains were more impaired, particularly in patients under 40 years old.

Conclusions:

  • Hemodialysis patients in this setting demonstrate overall functional independence by FIM and ADLQ.
  • The 7D5L scale is valuable for detecting subtle functional impairments in daily activities.
  • Motor and mobility domains require targeted interventions, and age impacts functional status.
  • Existing FS assessment tools may lack a time-dependent component crucial for HD patients.
