Replacing multiple daily insulin injections with exenatide BID in type 2 diabetes patients showed similar glycemic control but led to significant weight and waist circumference reduction. Exenatide may benefit obese patients with poor glycemic control.

- To compare the efficacy of substituting exenatide twice daily (BID) for MDI versus continuing MDI in patients with T2DM.

- 140 patients with T2DM on metformin and MDI were randomized to exenatide BID or continued MDI for 16 weeks.
- Assessed blood glucose, BMI, waist circumference, HbA1C, lipids, and side effects at baseline and 16 weeks.

- Glycemic control (HbA1C) was similar between exenatide and MDI groups (mean change -0.66% vs -0.74%, p=0.594).
- Exenatide group showed significant reductions in body weight and waist circumference (p<0.001) compared to the MDI group.
- Insulin requirements increased in the MDI group (5.86 units/day).
Conclusions:
- Exenatide BID can be an alternative to MDI in T2DM patients with obesity and poor glycemic control.
- Patients with long-standing diabetes, prolonged insulin use, or low C-peptide levels may not benefit from exenatide substitution.
