Background:

  • Diabetic pseudoperitonitis is a rare complication of type 1 diabetes mellitus, often presenting with severe ketoacidosis.
  • Its pathogenesis remains unclear, but it typically manifests as an acute abdomen, necessitating differential diagnosis from other acute abdominal conditions.

Observation:

  • A 46-year-old male presented with persistent epigastric pain, initially undiagnosed despite extensive investigations including imaging and endoscopy.
  • Peritoneal signs led to an indicated emergency surgery, which revealed no intra-abdominal pathology.

Findings:

  • Post-operative recognition of severe metabolic acidosis, initial hypoglycemia, glycosuria, ketonuria, and elevated glycated hemoglobin confirmed new-onset diabetes mellitus.
  • These findings, in conjunction with the clinical presentation, led to the diagnosis of diabetic pseudoperitonitis.

Implications:

  • Diabetic pseudoperitonitis can be the initial presentation of diabetes, mimicking surgical emergencies.
  • Awareness and prompt recognition of this rare condition are crucial to avoid unnecessary surgical interventions.
