    Summary

    A patient with IgG kappa myeloma experienced high serum phosphate levels without symptoms of hyperphosphatemia. This was caused by an increased phosphate content within the abnormal immunoglobulin G (IgG) protein.

    Background:

    • Multiple myeloma is a plasma cell malignancy.
    • Hyperphosphatemia can be caused by various factors, including pseudohyperphosphatemia.
    • Immunoglobulin abnormalities can affect laboratory test results.

    Observation:

    • A patient with IgG kappa myeloma presented with significantly elevated serum phosphate levels.
    • The patient exhibited no clinical signs or symptoms of hyperphosphatemia.
    • Standard laboratory assays indicated hyperphosphatemia.

    Findings:

    • The elevated serum phosphate was determined to be a laboratory artifact (pseudohyperphosphatemia).
    • This artifact resulted from an increased phosphate-to-protein ratio in the patient's serum IgG kappa myeloma protein.
    • The abnormal IgG kappa protein bound an excessive amount of phosphate per unit of protein.

    Implications:

    • This case highlights the importance of recognizing pseudohyperphosphatemia in patients with monoclonal gammopathies.
    • Accurate interpretation of serum phosphate levels requires consideration of underlying protein abnormalities.
    • Further investigation into the interaction between specific myeloma proteins and phosphate may be warranted.
