Background:

  • Pulmonary nodules (PNs) are frequently detected via thoracic computed tomography (CT).
  • Conventional diagnostic methods for PNs have limitations.
  • Electromagnetic navigation bronchoscopy (ENB) is increasingly utilized for diagnosing small PNs.

Purpose of the Study:

  • To evaluate the diagnostic yield of ENB for pulmonary nodules.
  • To assess the complication rate associated with ENB.
  • To determine ENB's utility in cases with low yield from traditional methods.

Main Methods:

  • Retrospective study conducted at United Christian Hospital, Hong Kong (April 2015 - June 2016).
  • ENB was offered to patients with low diagnostic yield from conventional flexible bronchoscopy (FB) and radial endobronchoscopic ultrasonography (R-EBUS).
  • Analysis of diagnostic yield and complication rates.

Main Results:

  • 99 patients underwent ENB, with 87.9% achieving specific pathology identification.
  • Malignant PNs were diagnosed in 67.7% of cases; tuberculosis (TB) in 14.1%.
  • Overall accuracy was 71.7%, with sensitivity 67.8%, specificity 100%. Complications occurred in 3.0% of patients (pneumothorax, bleeding, respiratory failure), with no mortality.

Conclusions:

  • ENB offers a significant diagnostic yield for pulmonary nodules not accessible by conventional FB/R-EBUS.
  • The complication rate of ENB is low.
  • ENB is a valuable tool for respiratory physicians and thoracic surgeons in diagnosing challenging PNs.
