Background:

  • Multiple sclerosis (MS) diagnosis can be challenging due to overlapping symptoms with other neurological conditions.
  • Cerebrovascular diseases, such as moyamoya disease, present with similar clinical and radiographic features to MS.
  • Distinguishing between MS and cerebrovascular disorders is critical for appropriate patient management.

Observation:

  • A 44-year-old woman presented with transient neurological symptoms and brain lesions suggestive of MS on MRI.
  • Magnetic resonance angiography revealed findings characteristic of moyamoya disease.
  • The patient subsequently experienced an acute ischemic stroke.

Findings:

  • Cerebral angiography confirmed moyamoya disease.
  • The patient received antiplatelet therapy.
  • Despite the stroke, the patient showed significant neurological improvement by discharge.

Implications:

  • Moyamoya disease should be considered in the differential diagnosis of patients presenting with transient neurological symptoms and atypical white matter lesions.
  • Comprehensive clinical evaluation and advanced neuroimaging are essential for accurate diagnosis.
  • Timely identification and management of cerebrovascular disorders can prevent severe neurological deficits.
