A new evidence-based algorithm for diagnosing periprosthetic joint infection (PJI) has been developed and validated. This diagnostic tool improves accuracy and can help reduce healthcare costs in revision arthroplasty cases.
Area of Science:
- Orthopedic Surgery
- Infectious Diseases
- Diagnostic Medicine
Background:
- Current periprosthetic joint infection (PJI) diagnostic guidelines lack validation and do not incorporate newer diagnostic methods.
- There is a need for an updated, evidence-based diagnostic algorithm for PJI.
Purpose of the Study:
- To develop and validate an evidence-based diagnostic algorithm for periprosthetic joint infection (PJI).
- To improve the accuracy and efficiency of PJI diagnosis in revision arthroplasty.
Main Methods:
- A multi-institutional study involving total joint arthroplasty patients.
- Infected patients (n=684) met Musculoskeletal Infection Society criteria; aseptic revision patients (n=820) served as controls.
- The algorithm was validated on a separate cohort of 422 cases.
Main Results:
- The algorithm recommends physical examination, inflammatory markers (CRP, ESR, D-dimer), and joint aspiration with fluid analysis.
- Alpha defensin showed no added benefit; intraoperative findings aid inconclusive cases.
- The proposed algorithm achieved high sensitivity (96.9%) and specificity (99.5%).
Conclusions:
- The validated, evidence-based algorithm provides guidance for diagnosing PJI in revision arthroplasty.
- Implementation of this algorithm can enhance clinical practice and potentially reduce costs.
