Summary

Benign prostate hypertrophy (BPH) may cause fibrotic changes and gland atrophy in the peripheral zone of the prostate. This finding could explain the observed inverse relationship between BPH and prostate cancer (PCa) incidence.

Background:

  • An inverse relationship exists between benign prostate hypertrophy (BPH) size and prostate cancer (PCa) incidence.
  • The underlying mechanisms linking BPH and PCa remain largely unexplored.
  • BPH-induced transition zone growth may cause peripheral zone fibrotic changes, leading to gland atrophy.

Purpose of the Study:

  • To investigate the histo-anatomical changes in the peripheral zone associated with BPH.
  • To quantitatively and qualitatively examine fibrotic changes and gland atrophy in the peripheral zone.

Main Methods:

  • Analysis of 39 radical prostatectomy specimens.
  • Measurement of peripheral fibrotic zone (capsule) thickness at six locations.
  • Assessment of gland atrophy within the fibrotic zone.
  • Multiple regression analysis to correlate prostate size with capsule thickness.

Main Results:

  • A strong positive correlation was found between prostate size and average capsule thickness (r=0.707, p<0.05).
  • Fibrotic changes in the peripheral zone were spatially associated with gland atrophy.
  • Glands within the fibrotic peripheral zone exhibited elongated and atrophic morphology.

Conclusions:

  • BPH is associated with the development of fibrotic material and gland atrophy in the peripheral prostate zone.
  • Peripheral zone gland atrophy, where most prostate cancers originate, may explain the inverse relationship between BPH and PCa.
  • These findings provide a potential mechanism linking BPH and reduced PCa incidence.
