Background:

  • Polypoid compound melanocytic nevi can present with features that challenge accurate diagnosis.
  • Atypia in these nevi may resemble melanoma, leading to potential overdiagnosis.

Purpose of the Study:

  • To prospectively review polypoid compound melanocytic proliferations.
  • To characterize atypical features and diagnostic challenges in these lesions.
  • To differentiate benign nevi from melanoma.

Main Methods:

  • Prospective review of 40 polypoid compound melanocytic proliferations without overt malignancy.
  • Histologic evaluation for architectural and cytologic atypia.
  • Molecular studies to assess melanoma-specific profiles.

Main Results:

  • Common atypical features included asymmetry, shouldering, poor circumscription, and adnexal extension.
  • Less common were severe cytologic atypia, dermal mitoses, and pagetoid spread.
  • All lesions had a benign dermal component; 5 were classified as atypical nevi, 2 as ambiguous proliferations.

Conclusions:

  • Polypoid compound melanocytic nevi can exhibit junctional atypia, often due to trauma and regeneration.
  • Pathologists must recognize these features to avoid overdiagnosis of melanoma.
  • Molecular studies confirmed no melanoma profiles in the evaluated lesions.
